GENERAL STATEMENT OF DUTIES
Under the direction of Project Concern’s 211/Information and Referral Director, the Homeless Program Coordinator works with clients, utilizes community resources to assist the client in obtaining emergency shelter or permanent housing, and coordinates the Phoenix Housing Shelter Plus Care Program. This position requires a well developed understanding of the human resource field and interpersonal relationships, particularly in the area of homelessness.
RESPONSIBILITIES INCLUDE, BUT ARE NOT LIMITED TO:
- Conducts initial needs assessments of individuals/families for homeless and housing services.
- Conducts outreach to homeless populations.
- Implements and evaluates the Continuum of Care community initiatives through regular meetings with the Homeless Advisory Board and Continuum of Care.
- Assists in distribution of Homeless Shelter Operating Grant (HSOG) funds.
- Maintains client files with Housing and Urban Development (HUD) and Homeless Management Information System (HMIS) documentation.
- Participates in rotation of on-call, after-hours crisis/homelessness coverage.
- Processes clients for and into the Shelter Plus Care (SPC) homeless program.
- Processes SPC rental applications and submit recommendations for placement to the I&R Dept. Director.
- Investigate and secure relationships with landlords and enter into contractual agreements for the SPC program.
- Maintains records and files for participating landlords.
- Regularly inspects units, enforces the rental agreement with the clients, and oversee maintenance/repairs of units.
- Enforces the rental agreement with SPC Participants, including submitting recommendation for lease termination/eviction to the I&R Director.
- Prepares SPC monthly expense worksheets and performs rent calculations and adjustments in accordance with HUD guidelines.
- Works closely with various groups from outside Project Concern, (i.e. case management, mental health and substance abuse providers) to provide effective service coordination
- Establishes suitable agreements between the City Housing Authority, various housing providers and participants to ensure compliance with all relevant rules and regulations.
- Arranges for suitable inspection of housing units for compliance with Housing Quality Standards.
- Completes any other duties as assigned.
QUALIFICATIONS
- Graduation from an accredited four year college or university with a Bachelor’s degree in Human or Social Services or a related field, and significant experience in a human service agency.
- A well developed understanding of the human resource field, with interpersonal relationship skills particularly in the area of homelessness.
- Ability to work with minimal supervision.
- Ability to work with a variety of individuals and groups.
- Ability to write grammatically correct, clear and concise materials.
- Ability to work courteously and responsively with the public.
- Well developed problem solving skills.
- Well developed written and verbal communication skills.
- Ability to adapt to changing situations and apply time management skills in these situations
- Strong skills in relation to organization, time management, and decision making.
- Experience and knowledge of Microsoft Work and Excel and ability to learn and utilize database software programs.
- Ability to maintain client and agency confidentiality.
- Transportation required.
- Ability to work with a variety of mentally, emotionally, physically or behaviorally challenged individuals, as well as a variety of organizations and cultures
PHYSICAL REQUIREMENTS
Possess natural or aided hearing adequate to provide regular phone interaction with broad range of persons. Possess natural or aided vision to support daily personal computer usage, regular standard office equipment and preparation and the interpretation of hand written information.
Ability to lift 25-30 pounds regularly when moving equipment, books and supplies.
Range of motion, dexterity and coordination requisite with the daily use of a personal computer system and standard office equipment.
